● If a patient’s deductible has been met, check the Deductible Met box. If they have a
coinsurance, then they will not pay anything at their appointment and will be mailed a bill
for their portion after charges have been processed.
○ Note: The Deductible Met box does not automatically check or uncheck itself and
must be updated manually when necessary.

Insured Party: The subscriber for the policy (the independent).
● If the insured party is someone other than the patient, you must input their name and
date of birth in the following window.

Additional Info: This section is used to note any other pertinent plan details, such as
● **If a referral and/or authorization is required, including what type, when, & how to
submit**
● The amount of the patient’s deductible that is remaining (if applicable) & the last date
checked
● Any visits that have been used of the maximum allowable for the policy year
● Phone number for Provider Services for obtain benefits
