how many lifetime visits the patient has had for the body part for which they are
being seen. *It is very important to flag and note in the patient’s chart that auth is
needed and after which number visit!! Add an authorization to the patient’s case
to alert the Coordinator and PT when auth must begin.
○ After the patient’s 8th lifetime visit per body part, auth must be requested by
faxing a completed MDIPA Rehab Extension Request Form with relevant clinical
notes.
● Other insurance provided through UHC:
○ GEHA: The ID number will include the letters GEHA following the numerical
portion.
■ Benefits phone number: 877-343-1887
■ Authorization is required through OrthoNet by faxing a completed
Therapy Request Form found HERE with clinical notes.
○ UMR: Provider Services phone number is 877-233-1800. You will receive a fax
with a passcode, which will be used to speak with a representative for benefits.

Secondary Insurance:
● Verify that the secondary insurance plan is active as you would with primary insurance.
