                                                                All  employees  working  at  least  25  hours  per  week  and  their

                                                                When you can enroll

            Dental Options                           6           Eligible employees may enroll at the following times:
                                                                •   During annual open enrollment for an effective date of July

                                                                   1, 2019 to June 30, 2020.

                                                                •   As  a  new  hire,  you  may  participate  in  the  company’s

                                                                   Medical,  Dental,  Vision,  Employee  Assistance  Program,
            Life and AD&D Insurance                                Disability, Voluntary Life, and the company-paid Basic Life
             - Basic                                 7             and AD&D plans on the first of the month following your

                                                                   Within 31 days of a qualified change in family status (see

                                                                The Medical HMO (employee only) is 100% employer paid and

                                                                the Dental DHMO or PPO (employee only), Vision (employee

                                                                cost and are paid 100% by Westmark School.  You have the

                                                                option to buy-up to the Medical PPO plan as well as dependent
                                                                coverage for the Medical, Dental and Vision plans.  The

            Employee Assistance Program (EAP)        8          Voluntary Life benefits you elect will be paid by you at group

                                                                Term Disability programs is mandatory. Benefits are paid

                                                                and Vision are deducted before taxes are withheld, which saves
