Disability Insurance
California State Disability
If you experience a Short Term Disability claim, you are eligible for benefits provided by the state. California State Disability
Insurance (SDI) is a partial wage-replacement insurance plan for California workers. The SDI program is state-mandated and funded
through employee payroll deductions. SDI provides short-term benefits to eligible workers who suffer a loss of wages when they
are unable to work due to a non-work-related illness or injury, pregnancy or childbirth. For more information on eligibility, benefit
amounts and instructions on how to file a claim for the SDI program, please visit the EDD website at www.edd.ca.gov.
CSI | Short Term Disability Insurance
CSI offers you Short Term Disability (STD) to provide income replacement if you become disabled due to accident, sickness or
pregnancy. If you experience a temporary disability, substantiated by a doctor’s off-work order, benefits begin 8 days after the start
of your disability and will continue as long as you remain disabled up to a maximum of 180 calendar days. California State Disability
provides a benefit of 60% of your pre-disability earnings (or 70% for those making less than 1/3 of the state’s average quarterly
wage) up to a maximum benefit set by the state of California. CSI’s plan provides a total of 80% of base salary, integrated with
California State Disability or Workers Compensation (as applicable).
Reliance Standard | Long Term Disability Insurance
CSI offers you Long Term Disability (LTD) to provide income replacement if you become disabled for an extended period of time,
through Reliance Standard. If you become totally and permanently disabled, benefits begin 180 days after the start of your illness
or injury and will continue as long as you remain disabled up to Social Security Normal Retirement Age. LTD works with state
disability programs, Social Security, and any other group disability coverage, to provide you with a combined monthly benefit equal
to the following:
• Class 1, Executives and Managers: 60% of your pre‐disability earnings up to a maximum benefit of $25,000 per month
• Class 2, 3 and 4, All Others: 60% of your pre‐disability earnings up to a maximum benefit of $10,000 per month

Reliance Standard | Travel Assistance Program
On Call is a 24-hour, toll-free service that provides a comprehensive range of information, referral, coordination and arrangement
services designed to respond to most medical care situations and many other emergencies you may encounter when you travel
more than 100 miles from home or in a foreign country. On Call also offers pre-trip assistance including passport/visa
requirements, foreign currency and weather information.

Reliance Standard | Bereavement Support Services
Bereavement Support Services provide help to you and your family members when coping with the loss of a loved one. This service
is provided at no cost to you. Should you or a family member experience the loss, this service offers access to unlimited and
confidential telephonic grief counseling, as well as legal and financial consultation when you need it most. Professional clinicians,
who are experienced in dealing with grief, are available to discuss any concerns and offer comfort.
