                                                CSI Family Health Center
         The onsite CSI Family Health Center (FHC) provides primary health care services to eligible employees and their dependents, with
         no  office  visit  or  lab  work  co-pays  (Health  Savings  Account  plan  members  pay  normal  costs  up  to  their  deductible).  The  FHC
         provides a wide variety of services including acute care for lacerations, contusions, sprains, strains, and flu and cold symptoms. FHC
         medical staff also provides health risk assessments (annual physicals) for employees and spouses, sports and school examinations
         for dependents, immunizations (such as flu shots) and health education for a wide variety of life style diseases. As a convenience,
         prescription medications can also be delivered directly to the FHC for you to pick up, whether or not the prescription is provided by
         an FHC physician.

         The FHS is professionally staffed by Kaiser Permanente and is available to all employees and dependents with coverage through
         CSI, regardless of plan choice.  Employees who have Kaiser coverage through their spouse’s employer can also utilize the FHC, but
         must pay the applicable co-pay through payroll deduction.
