This brochure provides a summary of your benefit options and
                                                                is designed to help you make your choices and enroll for your
                                                                Westmark  group  benefits  coverage.    If  you  would  like  more
                                                                information  about  any  of the  benefits  described  here,  please

                                                                Enrollment Information



                                                                Who may enroll
                                                                All  employees  working  at  least  25  hours  per  week  and  their

                                                                benefits program.  Your eligible dependents include:

                                                                The Medical HMO (employee only) is 100% employer paid and

                                                                cost  and  are  paid  100%  by  Westmark  School.    You  have  the

                                                                Voluntary Life benefits you elect will be paid by you at group

                                                                discounted  rates.  Participation  in  the  Short  Term  and  Long
