This brochure provides a summary of your benefit options and
is designed to help you make your choices and enroll for your
Westmark group benefits coverage. If you would like more
information about any of the benefits described here, please

Enrollment Information
Who may enroll
All employees working at least 25 hours per week and their

- Changes to enrollment 3 • During annual open enrollment for an effective date of July
1, 2020 to June 30, 2021.
• As a new hire, you may participate in the company’s

Medical, Dental, Vision, Employee Assistance Program,
Disability, Voluntary Life, and the company-paid Basic Life
Medical Options and AD&D plans on the first of the month following your

Dental Options 8 Paying for your coverage
The Medical HMO (employee only) is 100% employer paid and
Vision 9 the Dental DHMO or PPO (employee only), Vision (employee
only) and Basic Life and AD&D plans are provided to you at no
cost and are paid 100% by Westmark School. You have the
Life and AD&D Insurance
option to buy-up to the Medical PPO plan as well as dependent
- Basic 10 coverage for the Medical, Dental and Vision plans. The

Voluntary Life benefits you elect will be paid by you at group
discounted rates. Participation in the Short Term and Long
Disability Term Disability programs is mandatory. Benefits are paid
- Short Term Disability (STD) 10 out on a tax-free basis. Your contributions for Medical, Dental,
- Long Term Disability (LTD) 10 and Vision are deducted before taxes are withheld, which saves
you tax dollars and results in higher take-home pay. As a
Flexible Spending Accounts (FSA) result, the IRS requires that your elections remain in effect for
- Health Care Spending Account (HCSA) 11 the entire year.
