ELIGIBILITY & ENROLLMENT
Who may enroll
SC Fuels Team Members
• Regular, full-time team members working at least 30 hours per week - medical
• Regular, full-time team members working at least 32 hours per week - all other benefits
Dependents
• Legally married spouse (if other insurance is not available through his/her own employer - medical only)
• Dependent children under the age of 26, regardless of student or marital status
• Registered domestic partner of same sex or opposite sex (if other insurance is not available through his/her
own employer - medical only)
Required Information
At enrollment you are required to enter the Social Security Number for all covered dependents. Health Care
Reform law requires the company to report this information to the IRS each year to show that you and your
dependents have coverage and are not subject to a penalty. This information will be securely submitted to the
IRS and will remain confidential.
SC Fuels requires the following information in order to enroll your spouse or domestic partner:
• Spouse or Domestic Partner: Spouse/Registered Domestic Partner Eligibility Form must be completed and
turned into Human Resources
• Marriage certificate or domestic partner affidavit may be required
When you may enroll
Benefits Plan Year: As an Eligible Team Member:
April 1 - March 31 • As a new hire, you may participate in the company’s
benefits program on the first day of the month following
the completion of one month of full-time employment
• Each year, during open enrollment
• Within 30 days of a qualifying event as defined by the IRS
(see Changes To Enrollment on the following page)
• You may enroll in Voluntary Life, Voluntary AD&D,
Voluntary Short Term Disability, and Voluntary Long Term
Disability insurance at any time, subject to proof of good
health and carrier approval
