Arrowhead Credit Union offers you the opportunity to purchase Short Term Disability (STD) to provide
income replacement if you become disabled due to accident, sickness or pregnancy, through Mutual of
Omaha. If you experience a temporary disability, benefits begin 7 days after the start of your accident,
sickness or pregnancy. California State Disability provides a benefit of 60% of your pre‐disability earnings up
to a maximum benefit set by the state of California. This plan pays in addition to California State Disability an
additional 30% of your pre‐disability earnings up to a maximum benefit of $1,000 per week.

Benefit Waiting Period 7 Days
Weekly Benefit Level Class 1-4: 30% of Team Member’s Weekly Covered Earnings
Weekly Benefit Maximum Class 1-4: $1,000 per Week

Maximum Benefit Duration Class 1: 12 Weeks
Class 2: 24 Weeks
Class 3-4: 52 Weeks

Arrowhead Credit Union pays for your Long Term Disability (LTD) and offers it to provide income replacement
if you become disabled for an extended period of time, through Mutual of Omaha . If you become totally and
permanently disabled, benefits can begin after the below benefit waiting period. LTD works with state
disability programs, Social Security, and any other group disability coverage, to provide you with a combined
monthly benefit level as shown in the below chart up to the monthly benefit maximum.

Class 2: 180 Days
Class 3: 360 Days
Class 4: Not Covered
Monthly Benefit Level Class 1-2: 66-2/3% of Team Member’s Basic Monthly Earnings
Class 3: 60% of Team Member’s Basic Monthly Earnings
Class 4: Not Covered
Monthly Benefit Maximum Class 1: $12,500 per Month
Class 2: $7,500 per Month
Class 3: $2,000 per Month
Class 4: Not Covered

• Class 1: All eligible Executives working 38 or more hours each week.
• Class 2: All eligible Team Members earning $40,000 or more annually and working 38 or more hours each week.
• Class 3: All eligible Team Members earning less than $40,000 annually and working 38 or more hours each
week.
