         The Well-Being Assessment is a scientifically validated instrument that goes beyond the typical health-assessment survey
         because it measures productivity, overall outlook, emotional and physical health, healthy behaviors, work environment,
         and allows access to health/life resources. After you take the Well-Being Assessment, you will receive a well-being score
         and recommended daily challenges. The “My Score” data enables participants to better understand their current state of
         well-being, benchmarking their well-being data against national, state, and regional benchmarks.


         Daily Challenge offers quick actions, realistic content, and social networking, so you can have fun, while getting
         healthier. A daily message is sent with one simple activity or “challenge” and a reason why it will help improve
         well-being. There are dozens of tracks and topics to choose from, such as All-Day Energy, Educated Eating, Simple
         Cuisine, Money Matters, Stress Management, and Wise Worker.

         Health  Advocate  provides  you  with  experienced  help  when  dealing  with  benefit-related  issues.  Their  team
         includes highly trained registered nurses, backed by a staff of medical directors and administrative experts. When
         you call Health Advocate for assistance you are assigned a Personal Health Advocate who takes responsibility for
         helping you resolve problems. They will:
         •  Help you get your claims and/or billing issues resolved.
         •  Help  you  get  answers  to  medical  questions  about  test  results,  treatment  plans,  and  medications
            recommended by your doctor.
         •  Work with the insurance carriers to help obtain appropriate approval for services when necessary.
         •  Assist in communications between your doctors and the insurance companies.
         •  Help you locate and contact the best doctors, dentists, and treatment facilities in your networks.
         •  Share unbiased health information when you need to make an informed decision about your care.

         Their years of experience make it easy. You need to make only one call or send an email and your personal health
         advocate  will  act  quickly  and  effectively  on  your  behalf  to  protect  your  privacy  and  keep  information  strictly
         confidential, find the best answers, and make any necessary follow-up arrangements.
