Voluntary Benefits
You have a choice of four types of voluntary plans through Aflac that can help provide a financial cushion in
the case of an accident or illness.
• Plans are paid with pre-tax deductions from your paychecks
• No deductibles or copayments
• No network restrictions—you choose your own medical treatment provider
• No coordination of benefits—Aflac will pay you regardless of any other insurance you have
• No lifetime limit—policy won’t terminate based on number or dollar amount of claims paid
• Premiums for the plans will never increase and are locked in
• Plans are portable meaning you can keep your plans if you change jobs or retire as long as premiums are
paid
Aflac | Accident Advantage
This plan pays for any accidental injury on or off-the-job. This accident plan pays cash benefits for emergency
treatment either at your own doctor or at an emergency room. The plan includes cash benefits for follow-up
treatment, hospitalization, intensive care, surgery, physical therapy, MRI, x-rays, epidural pain management,
rehabilitation, prosthesis, ambulance, accidental death and dismemberment, and a wellness benefit. If the
injury is the result of an organized sporting event, receive an extra 25% cash benefit.
Aflac | Cancer Care Plan
The plan pays a cash benefit upon initial diagnosis of internal cancer, with a variety of other benefits payable
for treatments, such as chemotherapy, radiation, surgery, hospitalization and much more. The cancer plan is
designed to help cover expenses such as deductibles, experimental treatments, travel and lodging, and day-
to-day expenses—the choice is yours. The plan also includes wellness benefits for cancer screening exams.
Aflac | Critical Care
This plan provides a lump sum cash benefit upon diagnosis of a specified event such as a heart attack, stroke,
end-stage renal failure, coma, paralysis, major human organ transplant, sudden cardiac arrest, major third-
degree burns, coronary artery bypass surgery, coma and paralysis. It also provides cash benefits for
hospitalization and continuing care.
Aflac | Hospital Confinement
Hospital care and clinical services account for a little over half of the health care expenditures in the nation.
Even if it is planned, a trip to the hospital can be a little intimidating. Aflac’s newly upgraded hospital plan
pays $500 to $2,000 cash per hospitalization for injury or sickness and also includes cash for physician visits,
diagnostic exams, lab tests, invasive diagnostic exams, surgery benefits, hospital intensive care confinement
and more.
