Medical Plan Choices

ABC Company recognizes the need for protection against the financial burden of costly health care
expenses. We offer employees a variety of medical plans to choose from, depending on your preferences
and location. It is important for you to know how each medical plan works so that you can make the best
choice for you and your family. Please note that different locations have different medical plan options
available.

Medical Plans Available in All Locations

Anthem Blue Cross

Premier Preferred Provider Lumenos Health Savings
Organization (PPO) Plan Account (HSA) Plan

Anthem Blue Cross Premier PPO Plan
When you enroll in the Anthem Blue Cross Premier PPO Plan, you may obtain services
from any provider you wish. When you obtain services from providers who participate in
the Anthem Blue Cross PPO network, your benefits are greater. When you obtain services
from providers who do not participate in the Anthem Blue Cross PPO network, your
out-of-pocket expenses are higher.

Anthem Blue Cross Lumenos HSA Plan
The Anthem Blue Cross Lumenos HSA Plan is a high-deductible health plan with a health
savings account. You receive the highest level of coverage when you obtain services from
providers who participate in the Anthem Blue Cross PPO network. When you obtain
services from providers who do not participate in the Anthem Blue Cross PPO network,
your out-of-pocket expenses are higher.

This plan includes a high deductible and no coinsurance or copays apply until the deductible
is met. You may fund your deductible and other qualified health care expenses with pre-tax
contributions from a Health Savings Account (HSA). See Understanding Health Savings
Accounts on page 4 for more details.
