Anthem Blue Cross Classic HMO Plan
When you enroll in the Anthem Blue Cross Classic HMO Plan, you select a Primary Care
Physician (PCP) who coordinates and manages your health care services. Your PCP provides
routine care and refers you to specialists when necessary. You may choose a different PCP
for each family member. Services that are not referred by your PCP are not eligible for
coverage under the Anthem Blue Cross Classic HMO Plan, except in emergency situations.
Kaiser Permanente HMO Plan
When you enroll in the Kaiser Health Maintenance Organization (HMO) Plan, you must
obtain health care services from Kaiser providers and facilities. Most medical services are
covered at 100% after a small copay when you receive care from a Kaiser provider. Services
not obtained from Kaiser providers or facilities will not be covered except in emergency
situations or with an authorized referral.
Kaiser Permanente HSA Deductible HMO Plan
When you enroll in the Kaiser Permanente HSA Deductible HMO Plan, you must obtain
health care services from Kaiser providers and facilities. The Kaiser Permanente HSA
Deductible HMO Plan is a high-deductible health plan with a health savings account. This
plan includes a high deductible and no copays apply until the deductible is met. You may
fund your deductible and other qualified health care expenses with pre-tax contributions from
a Health Savings Account (HSA). See Understanding Health Savings Accounts on page 4
for more details.

Exclusive Classic (EPO) Plan
Anthem Blue Cross Exclusive Classic (EPO) Plan
When you enroll in the Anthem Exclusive Classic (EPO) Plan, you may obtain services
from any provider you wish. When you obtain services from providers who participate in
the Anthem Blue Cross PPO network, your benefits are greater. When you obtain services
from providers who do not participate in the Anthem Blue Cross PPO network, your
out-of-pocket expenses are higher.
