DISABILITY INSURANCE

RELIANCE STANDARD | VOLUNTARY SHORT TERM DISABILITY
You have the option to purchase additional short term disability benefit that will pay in addition to your CASDI benefits. California
employees may purchase a policy of 20% up to $1,000 per week of income replacement. This benefit is payable after 7 days of sickness or
an accident. This plan is designed to provide greater income protection (effectively 80% with CASDI) and the benefit is tax free. With the
Reliance Standard plan, it will pay an additional 20% to give you 80% of income replacement.

Note: this plan has a pre-existing condition limitation and will not pay benefits for any illness diagnosed 3 months prior to the effective
date until you are insured for 12 months.

RELIANCE STANDARD | LONG TERM DISABILITY

Total Vision offers you Long Term Disability (LTD) to provide income replacement if you become disabled for an extended period of time,
through Anthem Blue Cross. If you become totally and permanently disabled, benefits begin 90 days after the start of your illness or
injury and will continue as long as you remain disabled up to Social Security Normal Retirement Age. LTD works with state disability
programs, Social Security, and any other group disability coverage, to provide you with a combined monthly benefit equal to 60% of your
pre‐disability earnings up to a maximum benefit of $10,000 per month.

ACCIDENT INSURANCE

RELIANCE STANDARD | VOLUNTARY ACCIDENT INSURANCE

Accident insurance pays you a lump sum benefit if you experience a covered accident. Some covered accidents include emergency room
visits, burns, hospital confinement, lacerations, dislocations and fractures. You will be paid a different lump sum depending on the severity
of some accidents. If your accident results in death or dismemberment, you or your loved ones will be paid accidental death &
dismemberment (AD&D) benefits. This plan includes a wellness benefit if you have your annual preventive care check up completed. There
