Wellness Program
Participating in a wellness program goes a long way in improving and maintaining your health. Taking a health assessment
will pinpoint any high-risk areas that could affect your health. Armed with that knowledge, Orriant’s health coaches will
be able to offer support and inform you of ways to lower your risks. Employees and spouses who choose to participate
will each pay $60 less per month on their health insurance premiums than those who choose not to participate.
To Participate:
1. Take a Health Assessment: Log into www.myorriant.com or call (888) 346-0990 to schedule an
assessment, or visit a certified health professional to complete an “independent assessment
form”. You will be asked to fast for 12 hours prior to taking your assessment.
2. Health Standard Check: If you didn’t meet the established Health Standard during your health
assessment and you have not previously worked with an Orriant Health coach, you will be
scheduled for an Orriant Health Plan (an initial phone conversation with your coach).
3. Complete the Snapshot: Access your online Orriant Wellness Portal after your Orriant Health
Assessment at www.myorriant.com and complete the snapshot. Paper copies are available
upon request.
Participation must be completed by April 20, 2018. Detailed instructions will be provided by the
Human Resources Department.
Continued Participation
If you meet the Health Standard, you must complete at least two qualified health promotion activities per quarter. These
activities include onsite RauxaFit activities, health education webinars, healthy recipes, workout challenges, health coach
contacts, and submitting proof of continued engagement in your own health and wellness.
If you did not meet the Health Standard, you must perform the following participation requirements to maintain enrollment:
• Work with coach to identify areas of wellness important to the participant to address based on their personal long
term goals.
• Develop a personalized action plan containing at least two wellness goals with the coach in which the participant
will track at least 75% of the time and reach at least 50% participation to maintain compliance.
• Maintain ongoing contact with assigned coach at a frequency determined between the participant and the coach.
Important:
Please note, if it is unreasonably difficult due to a medical condition for you to achieve the standards for the reward under
our wellness program, or if it is medically inadvisable for you to attempt to achieve the standards for the reward under
this type of a program, Rauxa will work with you to develop another way to qualify for the reward.
