SUPPLEMENTAL BENEFITS
Unum | Critical Illness
Parsons offers you the opportunity to purchase Critical Illness insurance at discounted group rates, provided by Unum. Critical
Illness insurance is designed to help you and your family offset the financial effects of a catastrophic illness with a lump sum benefit
if you are diagnosed with a covered critical illness. The Critical Illness benefit is based on the amount of coverage in effect on the
date of diagnosis of a critical illness or the date treatment is received according to the terms and provisions of the policy.
Covered illnesses include cancer, carcinoma in situ (25%), heart attack, coronary artery bypass surgery (25%), stroke, end stage
renal (kidney) failure, major organ failure, permanent paralysis as the result of a covered accident, coma as the result of severe
traumatic brain injury, blindness, and benign brain tumors. Additional covered conditions for dependent children include cerebral
palsy, cleft lip or palate, cystic fibrosis, down syndrome, and spina bifida.
Employee
You may purchase coverage for yourself in the following amounts: $10,000, $20,000, $30,000
Spouse or Domestic Partner ages 17-64
You may purchase coverage for your spouse in the following amounts: $5,000, $10,000
Child(ren) up to age 26
If you buy coverage for yourself, you will automatically be provided with a child benefit equal to 25% of your employee election.
All election amounts are guarantee issue during the initial enrollment period with no medical underwriting required. Future in-
creases in benefits or new enrollments will be subject to health questions.
Unum | Hospital Indemnity
Parsons offers you the opportunity to purchase Hospital Indemnity insurance at discounted group rates, provided by Unum. Group
Hospital Indemnity insurance is designed to help provide financial protection for you by paying a benefit due to a hospitalization
and in some cases, for treatment received for an accident or sickness, even if that treatment occurs outside the hospital. You can
use the benefit to meet the out-of-pocket expenses and extra bills that can occur. Should you or a covered dependent become
hospitalized, indemnity lump sum benefits will be paid directly to you in the amount of $1,500, regardless of the actual cost of
treatment. Coverage is available for you, your spouse and/or your dependent children. Please note, you must elect coverage for
yourself to elect coverage for your spouse or children.

will be subject to health questions.
Receive up to $150 Wellness Benefit from Unum!
As an enrolled member of the Critical Illness or the Hospital Indemnity plan, if you also participate in Parsons’ biometric
screening process, or if you have one of the listed wellness exams performed by your Physician, you will be eligible to receive
a $75 Wellness benefit from Unum if you purchase the Critical Illness or Hospital Indemnity plan. To receive your wellness
benefit from Unum simply follow the steps below:
• Enroll in the Critical Illness or Hospital Indemnity plan(s) in bswift
• Current Employees during 2017 Open Enrollment: Complete the Unum Express Wellness Claim Form located in
bswift and submit to Unum on/before December 31, 2016.
• New Hires (after 2017 Open Enrollment): Once completing the Parsons’ biometrics or having one of the
qualifying wellness exams performed by your Physician, simply call Unum to confirm completion of screening:
