Disability

If you are unable to work due to an illness or injury, our disability plans will work together to provide a source of
income to meet your needs. Disability insurance provides benefits that replace part of your lost income when you
become unable to work due to a covered injury or illness.

You must work 30 hours or more per week to be Maximum Benefit Duration To Social Security
eligible for Long Term Disability. Normal Retirement Age

Evidence of Insurability:
If you are no longer in your initial eligibility period, you may enroll in Voluntary Long Term Disability insurance
anytime during the year as long as you provide proof of good health. To provide proof of good health, you will be
asked to complete and submit a health questionnaire (Evidence of Insurability) and are subject to insurance
carrier approval. Guardian may approve or decline coverage based on a review of your health history. Your
requested coverage is not guaranteed until you have submitted the Evidence of Insurability (EOI) and been
approved by Guardian.

College Tuition Rewards
You can create your rewards account and start accumulating your Tuition Rewards
that can be used to pay up to one year’s tuition at over 380 private colleges and
universities across the nation.

Here’s how it works:
 Annual enrollment in this plan earns you 2,000 Tuition rewards (1 reward = $1 in
tuition reduction at a network of private colleges and universities).
 Guardian Dental participants receive a bonus after year four.
 These rewards are yours for your lifetime and can be given to children,
grandchildren, nieces, nephews, and godchildren.
 Sponsored students must be registered before August 24th of the year they enter 11th grade.
