may participate in the company’s benefits program. Your eligible dependents include:
• Your legally married spouse
• Your registered domestic partner (as defined by the state of California)
• Your children, stepchildren or children of your registered domestic partner to age 26, regardless of marital or student
status
• Any children for whom you are required to provide coverage under a Qualified Medical Child Support Order
• Your unmarried children, step-children or children of your registered domestic partner of any age, if they are
incapable of self-care due to a physical or mental disability
When You Can Enroll
As an eligible employee, you may enroll at the following times:
• As a new hire, you must enroll in PlanSource within 30 days from your date of hire. Your benefits will begin the first
of the month following one month of continuous employment with Career Group Companies
• Each year, during open enrollment
• Within 30 days of a qualifying life event as defined by the IRS (see Changes To Enrollment below)
Paying For Your Coverage
The Basic Life/Accidental Death & Dismemberment (AD&D), Long Term Disability, and Employee Assistance Program
(EAP) benefits are provided at no cost to you and are paid entirely by the Company. You and the Company share in the
cost of the Medical, Dental, and Vision benefits you elect. Any medical, dental, vision or flexible spending account
benefits that you elect will be paid by you and deducted before taxes are withheld which saves you tax dollars, resulting
in more take home pay for you. You cannot drop or change coverage unless you experience a qualifying life event.
