Medical Plan Highlights
Anthem Blue Cross | EPO Plan
With the Exclusive Provider Organization (EPO) plan, you may self-refer to any physician within the Anthem EPO
network and do not need to select a PCP. This plan requires that you only utilize doctors, clinics, and hospitals that
belong to the network, except in the case of an emergency. If you choose to see a provider who is not part of the
network, you will be required to pay the full cost for your care.
Anthem Blue Cross | PPO Plan
The Preferred Provider Organization (PPO) plan allows you to direct your own care. If you receive care from a physician
who is a member of the Blue Cross PPO (Prudent Buyer) - Larger Group network, a greater percentage of the entire
cost will be paid by the insurance plan. However, you are not limited to the physicians within the network and you may
self-refer to specialists. If you obtain services using a non-network provider, please note that you will be responsible for
the difference between the covered amount and the actual charges, and you may be responsible for filing claims.

• Go to www.anthem.com/ca/find-doctor
• Select Search as a Guest by Selecting a Plan Click Continue
• What type of care are you searching for? Select Medical
• Select your state
• What type of plan do you want to search with? Select Medical - Employer

• EPO Plan: Select National PPO (BlueCard PPO)
• PPO Plan: Select National PPO (BlueCard PPO)
