Benefits
Life and AD&D Insurance
UNUM| Basic Life
Life insurance protects your family or other beneficiaries in the event of your death while you are still actively employed with JFSLA.
Jewish Family Service of Los Angeles pays for coverage, offered through UNUM, in the amount of 2 1/2 times your basic annual
earnings to a maximum benefit of $800,000.
UNUM| Voluntary Life and AD&D
In addition to the company provided Basic Life and Accidental Death & Dismemberment (AD&D) benefits, you may elect to
purchase additional Supplemental Term Life and Accidental Death & Dismemberment (AD&D) insurance at discounted group rates
with Guarantee Issue (no Medical Underwriting) at the initial offering. You pay for this coverage on a post-tax basis through
convenient payroll deductions and the coverage is portable.
If you enroll at your initial eligibility period and purchase a minimum of $10,000 for Employee and $5,000 for your Spouse, you
will have the ability to increase up to the GI, no questions asked. Otherwise, if you decline coverage, in the future an Evidence of
Insurability (EOI) form is required if applying for benefit amounts above the Guarantee Issue. This health questionnaire is subject to
insurance carrier approval. The insurance company may approve or decline coverage based on a review of your health history.
Employee
You may purchase supplemental life insurance coverage for yourself in increments of $10,000 up to a maximum of $500,000, not
to exceed 5 times your annual earnings.
Spouse
You must buy coverage for yourself in order to purchase coverage for your eligible spouse. Benefits for your spouse are available
in increments of $5,000 to a maximum of $250,000 and may not exceed 100% of the Employee elected amount.
Child(ren)
You must buy coverage for yourself in order to purchase coverage for your eligible dependent child(ren). Benefits for your children
6 months or younger are $250 and children ages 6 months to 26 years are $10,000.

Guarantee Issue
Guarantee issue is a pre-approved amount of coverage that does not require you to provide proof of good health, and is available
to you during your initial eligibility period (upon hire). If you are no longer in your initial eligibility period, you must provide proof
of good health, you will be asked to complete a health questionnaire and are subject to insurance carrier approval. Unum may
approve or decline coverage based on a review of your health history. Guarantee issue is available in the following amounts for
Unum Voluntary Life and AD&D:
Employee = $200,000
Spouse = $25,000
Child(ren) = Entire Benefit Amount
