        Supplemental Benefits

        UNUM | Voluntary Critical Illness

        The  Unum  Critical  Illness  insurance  can  help  you  protect  yourself  and  your  family  from  the  unexpected  cost  of  fighting  a  life-
        threatening illness. You’ll receive a lump-sum cash payment upon diagnosis of a covered condition such as a Heart Attack, Stroke,
        Kidney Failure and Cancer.

         Coverage                                      Benefit



         Critical Illness with Cancer Coverage         Lump sum: Cash benefit to cover out of pocket expenses associated with a
                                                       specified critical illness

         Benefits                                      • Benefit can be paid for each covered condition
                                                       • Coverage is portable •Dependent children are automatically covered at
                                                       25% of the employee benefit amount

                                                       • Employee benefit choice of $5,000 or $15,000
                                                       • Spouse benefit of $5,000

                                                       • Rates are based on benefit amount elected, age, and tobacco/non-tobacco
                                                       status and remain unchanged for the duration of the policy

                                                       Additional diagnosis benefit
                                                       • Multiple payouts automatically included in the plan design
                                                        • Each condition payable once per lifetime per covered individual
                                                       • Additional benefits payable for diagnosis of another critical illness if sepa-
                                                       rated by 90 days or more and medically unrelated

                                                       Lump sum: Cash benefit to cover out of pocket expenses associated with a
                                                       specified critical illness
         Wellness                                      • Each covered individual will automatically receive the wellness benefit,
                                                       which will pay $50 per calendar year per insured individual  if a covered
                                                       health screening test is performed.
                                                       • Insured individuals are eligible for benefits 30 days after the effective date
                                                       of coverage.
                                                       • Sample employee semi-monthly premium for a $5,000 benefit with cancer

                                                              Wellness (non-tobacco)
                                                                  - 30 year old- $3.10
                                                                  - 40 year old- $5.28
                                                                  - 50 year old- $9.30
