Disability Insurance

UNUM| Long Term Disability
Jewish Family Service of Los Angeles offers you Long‐Term Disability (LTD) income replacement if you are disabled for an extended
period of time, through UNUM. Benefits begin 90 days after the start of your illness or injury. Long Term Disability works with
California State Disability, Social Security, and any other group disability coverage, to provide you with a combined monthly benefit
equal to 66.67% of your pre‐disability earnings to a maximum of $15,000 a month.
Supplemental Benefits

You may purchase individual policies from UNUM including Voluntary Life, Group Hospital Indemnity, Accident, and Critical Illness
with Cancer. Your premiums are paid through payroll deductions on a post-tax basis. UNUM policies offer cash payouts that go
direct to the policy holder to help cover what other insurance doesn’t. The policies are portable which means that you can keep
them at the current group rates, should you change jobs or retire.
UNUM | Voluntary Accident

The Unum Accident insurance will pay you a benefit outlined in the benefit summary to help cover your out of pocket medical
costs. For covered accidental injuries, fixed benefits are paid directly to you regardless of your other coverages and you can spend it
any way you choose. Benefits are paid according to a fixed schedule that includes benefits for hospitalization, fractures and disloca-
tions, emergency room visits, and more.

Coverage Benefit

Group Accident Lump sum: Cash benefit to cover out of pocket expenses associated with an
accidental injury

Benefits • Covered injuries include: broken bones, cuts, burns, eye injuries, ruptured
discs, etc.

UNUM | Hospital Indemnity

The Unum Hospital Indemnity Insurance will pay you a benefit outlined in the benefit summary to help cover your out of pocket
medical costs. The UNUM plan is being provided to help ease the financial impact of a hospitalization. The benefit provides a lump
sum payment in the event that you are admitted to the hospital. You can use the payment as you see fit. It can be used to help
cover your health insurance deductibles, copays, incidental hospital charges, etc.
