DISABILITY INSURANCE

GUARDIAN | VOLUNTARY SHORT TERM DISABILITY
SC Fuels offers you the opportunity to purchase Voluntary Short Term Disability (STD) income replacement through
Guardian at discounted group rates to provide protection if you are disabled for a short period of time. Short Term
Disability benefits begin 7 days after your accident, sickness or pregnancy-related disability. Short Term Disability
works with Social Security and any other disability coverage, to provide you with a combined weekly benefit equal
to 55% of your pre-disability earnings to a maximum of $1,173 per week for up to 13 weeks.

GUARDIAN | VOLUNTARY LONG TERM DISABILITY
SC Fuels offers you the opportunity to purchase Voluntary Long Term Disability (LTD) income replacement at
discounted group rates, through Guardian. If you become totally and permanently disabled, benefits begin 90 days
after the start of your illness or injury and will continue as long as you remain disabled up to Social Security Normal
Retirement Age. LTD works with state disability programs, Social Security, and any other group disability coverage,
to provide you with a combined monthly benefit equal to 60% of your pre-disability earnings to a maximum benefit
of $10,000 per month.

Pre-existing Condition Exclusion: If you were diagnosed, consulted or treated for any medical condition 3 months
prior to your enrollment in either of these plans, the disability caused by this condition will not be covered until
you have been on the plan for 12 months. After 12 months of consecutive enrollment, the pre-existing condition
exclusion will no longer apply.

NOTE: You may enroll in Voluntary Short Term and Long Term Disability anytime during the year as long as you complete and
return a health questionnaire to Guardian. Guardian may approve or decline coverage based on a review of your health history.
