NOTE: IRS W-2 Reporting Requirements Section 9002(a) of PPACA provides that employers must disclose the aggregate cost
of applicable employer-sponsored coverage provided to employees on Form W-2. Your W-2 will reflect the aggregate cost of
the medical plan and all pre-tax supplemental AFLAC plans in Box 12 as code DD.

Online enrollment with benefitsCONNECT is simple, secure and
can be done in a few minutes from any computer with internet
access. After enrolling online, you will have access to your benefit
information 24 hours a day, from any computer. For your security,
benefitsCONNECT is 128-bit encrypted and password protected.

Getting Started
Visit https://www.benefitsconnect.net/scfuels during the initial
set-up process. Be prepared to provide your social security
number as well as your dependent’s social security numbers
and birth dates. Initially your user name and password will be
defaulted to a standard format. Upon completing your first login
you will be prompted to change your password.
Example:
Employee Name: Matt Sample
Your user name is made up of the first six letters of your last Social Security Number: 123-12-1234
name, followed by your first initial and the last four numbers of User Name: samplem1234
your social security number. The initial password for the system Password: 123121234
is your social security number (without dashes).
