SUPPLEMENTAL BENEFITS

AFLAC | ACCIDENT
SC Fuels offers you the opportunity to purchase Voluntary Accident Insurance from Aflac at discounted group rates.
This plan offers cash payouts to help cover what other insurance does not.
• Covered injuries include broken bones, cuts, burns, eye injuries, fractures, etc.
• This plan pays a lump sum benefit based on type of injury sustained and treatment needed
• There are no health questions to answer
• Coverage for your entire family is available
• Coverage is portable at the same group rate
• A wellness benefit is included for each covered family member per year; this benefit builds each year from $25
to $75 after 6 years

AFLAC | CRITICAL ILLNESS WITH CANCER
SC Fuels offers you the opportunity to purchase Voluntary Critical Illness Insurance from Aflac at discounted group
rates. This plan offers cash payouts to help cover what other insurance does not. Covered illnesses include heart
attack, cardiac arrest, major organ transplant, stroke, cancer, and more.
• This plan pays a lump-sum benefit at the diagnosis of a covered illness
• You may choose benefit amounts: $10,000, $15,000, $20,000
• If elected, your spouse’s coverage can be 50% of your benefit amount and children’s coverage can be 50% of
your benefit amount
• Children automatically included at no additional fee
• Your benefit increases during the first 5 years of coverage by 5% at no additional cost
• You have a $200 mammography benefit paid once per calendar year per insured
• Coverage is portable at the same group rate
• If you do not elect this coverage during your initial eligibility, you must provide proof of good health to Aflac
for review to be accepted under the plan

AFLAC | HOSPITAL INDEMNITY
SC Fuels offers you the opportunity to purchase Voluntary Hospital Indemnity from Aflac at discounted group
rates. This plan offers cash payouts to help cover the unexpected costs associated with hospital admission and
confinement due to accident or sickness.
• This plan pays a per day benefit directly to you based on the type and length of hospital stay
• Covered admissions include hospital stays up to 31 days / hospital intensive care unit stays up to 10 days
• This plan will work with your HSA plan to help with any deductible expenses
• You have a $100 mammography benefit, and a $50 wellness benefit paid once per calendar year per insured
• There are no health questions to answer
• Coverage is portable at the same group rate
• Coverage for your entire family is available
