Guardian PPO Dental Plan
The Guardian Preferred Provider Organization (PPO) Dental plan covers a wide range of dental services. You may
visit a PPO dentist and benefit from the negotiated rate or visit a non-network dentist. When you utilize a PPO
dentist, your out-of-pocket expenses will be less. You may also obtain services using a non-network dentist;
however, you will be responsible for paying the difference between the covered amount and the actual charges
and you may be responsible for filing claims.

*Dentists who are out-of-network have not agreed to pricing, and may bill you for the difference between what Guardian pays them and what
the dentist usually charges.

Maximum Rollover
Guardian will roll-over a portion of your unused annual maximum into your personal Maximum Rollover Account
(MRA). If you have a paid claim and do not exceed the paid claims threshold of $700 during the benefit year, you
will be credited $350 to your MRA. As an added advantage, more money will be rolled over if in-network dentists
are used. The MRA also has an account limit of $1,250.

College Tuition Rewards
When you enroll in the Guardian Dental plan through Burnham, you can create a College Tuition Rewards account
and start accumulating rewards that can be used at over 400 participating private colleges and universities across
the nation. Here is how it works:
• Enrollment in this plan earns you 2,000 tuition rewards annually, per product.
• Each tuition reward point equals $1 in tuition reduction.
• You will receive a bonus of 2,500 points after year four.
• These rewards are yours for your lifetime and can be given to children, grandchildren, nieces, nephews and
godchildren.
