Aetna HSA Plan
• Similar to the OAMC POS plan, you may access care from any doctor you wish. You will receive the greatest
coverage when using network providers. When you use non-network providers, your coverage will be less,
you will be responsible for the difference between the covered amount and the actual charges, and you may
be responsible for filing claims.
• This plan is considered to be a high deductible health plan (HDHP) linked to a special, tax-qualified health
savings account (HSA). When you make contributions to an HSA, you reduce your taxable income and save
money on Federal income taxes.
• You may use your HSA funds to pay for current eligible health care expenses or save toward future
expenses.
• When you open an HSA account through our preferred vendor, PayFlex, Burnham makes a monthly
contribution into your HSA account:
– Employee: $50/month
– Employee + Spouse or Child(ren): $75/month
– Employee + Family: $100/month
• In addition to the company contributions, you may elect to make contributions into your account up to the
IRS maximums shown on page 20.
Aetna Online, Mobile and Phone Access
Manage your care online by registering at www.aetna.com. You can locate network providers,
manage your claims, obtain health and wellness information, and much more.
Once you’ve registered, download the Aetna Mobile App, available on the App Store and
Google Play for on-the-go convenience.
You can reach customer service for the Aetna HMO at 800-445-5299 (refer to the “HMO”
network).
Customer service for the Aetna OAMC POS and HSA plans is available at 877-204-0431 (refer
to the “Managed Choice POS (Open Access)” network).
