tools and resources to help you achieve optimal health and well-being. Our health and wellness program is designed for
your unique needs whether you are suffering from asthma, expecting a baby or just fighting a cold, we offer ways to help.

ConditionCare
If you have asthma, diabetes, chronic obstructive pulmonary disease (COPD), heart disease or heart failure, ConditionCare
can give you the tools and resources you need to take charge of your health. You will have access to to nurse cre managers,
dietitians and other health care professionals to help you reach your health goals.
Future Moms
Having a baby is an exciting time for all in the family. Future Moms can help you have a healthy pregnancy and a healthy
baby. Sign up as soon as you know you are pregnant. You will receive support from a nurse specializing in obstetrics who
can answer your questions and will call to check on your progress.
24/7 NurseLine
Whether its 3 a.m. or a lazy Sunday afternoon, you can talk to a registered nurse any time of the day or night. NurseLine will
answer questions about health concerns and help you decide where to go for care when you doctor is not available.

Did you know that different facilities may charge different amounts for the same service? With the Estimate Your Cost tool,
you can estimate your share of the costs before you receive your care. Compare facilities based on their quality measures for
certain procedures — length of stay, patient experience, complications and more. Estimate Your Cost is just one of the many
tools Anthem offers to help you manage your health care, simply and conveniently.

To Estimate Your Cost
Go to www.anthem.com/ca and log in (located in the top right corner). Select Estimate Your Cost. Simply search or
browse for the procedure you are looking for and the tool will help guide you. You can easily compare facilities in
your area.
