Disability Insurance





         Anthem | Voluntary Short Term Disability Insurance

         9 out of 10 disabilities are caused by illness, not accidents, and every disability claim is a medical or behavioral event requiring a
         visit with a doctor. So it’s important that your disability and medical plans communicate with each other. CHSI offers you the op-
         portunity to purchase Voluntary Short Term Disability (STD) income replacement at discounted group rates, through Anthem Blue
         Cross. If you experience a temporary disability, benefits begin 7 days after the start of your accident, sickness or pregnancy and will
         continue as long as you remain disabled up to a maximum of 25 weeks. STD works with state disability programs, Social Security,
         and any other group disability coverage, to provide you with a combined weekly benefit equal to 67% of your pre‐disability earn-
         ings up to a maximum benefit of $1,400 per week.

         Pre-existing conditions: A pre-existing condition is an illness or injury for which you received treatment or where symptoms were
         present within 3 months prior to your effective date of coverage. A disability that begins in the first 12 months after your effective
         date will not be covered if it results from a pre-existing condition.

         If your application is submitted to Anthem more than 31 days after you became eligible, you must submit evidence of insurability
         and Anthem must approve it in writing.

         Anthem | Long Term Disability Insurance

         If your disability extends beyond the STD benefit period, you may qualify for Long Term Disability (LTD) benefits. In the event you
         become disabled for an extended period of time, LTD benefits will begin on the 181st day following the start of your illness or inju-
         ry and will continue as long as you remain disabled up to Social Security Normal Retirement Age. LTD provides a benefit equal to
         60% of your pre-disability earnings up to a maximum benefit of $6,000 per month.
