Voluntary Benefits




         Trustmark | Accident Insurance
         Accident Insurance covers unexpected expenses that result from all kinds of covered accidents.  You can use the benefit to
         help  pay  your  health  insurance  deductible  or  to  cover  out-of-pocket  expenses.  You  are  not  required  to  answer  health
         questions  and  there  are  no  pre-existing  condition  limitations.  This  plan  is  available  for  you,  your  spouse  &  dependent
         children. This plan includes a health screening benefit of $100 per person per year for completing certain health screening
         exams.

         Trustmark | Critical Health Events

         Critical Health Events will pay a lump-sum benefit upon the diagnosis of a covered critical illness such as a heart attack,
         stroke, cancer or end-stage renal failure. With the Critical Health Events plan, benefits are paid for early detection as well as
         for later-stage diagnosis.  Replenishing annual amounts allow you to remain covered for new or reoccurring condition(s).
         Depending on the diagnosis, your benefit payment will be 100%, 50% or 10% of your selected benefit amount, not to exceed
         the  annual  maximum  available.  This  plan  includes  annual  health  screening  benefits  of  $100  for  one  routine  plus  one
         diagnostic exam, a $25 benefit for biometric screening, as well as $250 for genetic cancer testing payable one time.


         Trustmark | Universal Life w/Long Term Care Insurance
         There are two options for permanent life insurance with Trustmark—Traditional Universal Life and Universal LifeEvents. They
         both include a life insurance benefit and a long-term care benefit that can help pay for home healthcare, assisted living,
         adult daycare or nursing home services at 4% per month for up to 25 months.

         The major difference with the Universal LifeEvents plan is that it pays a higher death benefit during your working years when
         expenses are high. At age 70, when financial needs are typically lower, the death benefit reduces to one third the original
         amount. The LTC benefit does not reduce, this benefit continues throughout retirement to match the greater need for LTC.
         This  makes  the  Universal  LifeEvents  plan  more  affordable  at  time  of  purchase  than  the  Traditional  Universal  Life  plan.
         However, the Universal LifeEvents is only offered to employees that are under age 64, while the Traditional Plan is available
         to age 75 for employees and age 70 for spouse.





         Enrollment Services– VBA Benefit Counselors












                     To Enroll with VBA as a new hire:
                     All eligible employees should meet with a benefits educator by phone to elect or waive benefits. Employees will call
                     Voluntary  Benefit  Advisors  Service  Center  using  the  phone  number  referenced  below  to  speak  with  a  benefits
                     educator. The benefit educator will be able to answer questions you have about all of the benefits offered to you and
                     enter your enrollment information over the phone.  If you are covering dependents, please have their social security
                     number and birth dates available during your enrollment session.  Please note:  Employees who do not want to enroll
                     in  the  company  benefits,  must  still  call  the  VBA  Service  Center  to  decline  benefits.  VBA  can  also  provide  claims
                     assistance for your Trustmark Accident, Critical Health Events, and Universal Life plans.
