Supplemental Benefits
You may purchase additional insurance from Unum that will help to cover additional out-of-pocket expenses. These policies
offer direct-to-the-policyholder cash payouts to help cover what other insurance doesn’t. The policies available include;
Accident, Critical Illness with Cancer, and Hospital Indemnity insurance. Your premiums are paid through payroll deductions
on an after-tax basis. All of the Unum policies are portable, which means that you can keep them should you change jobs or
retire, with no increase in premiums. Please refer to EaseCentral for benefit details and rates.
Unum | Accident
Unum Accident insurance will pay you a benefit to help cover your out-of-pocket medical costs in case of an accident. For
covered accidental injuries, fixed benefits are paid directly to you, regardless of any other coverage. Benefits are paid
according to a fixed schedule that includes benefits for hospitalization, fractures and dislocations, emergency room visits,
major diagnostic exams, physical therapy, and more. This plan will pay benefits for an off-the-job accident. This plan is
guarantee issue and does not have any pre-existing condition limitations. This plan pays a $50 annual wellness benefit for
covered employees and spouses for getting a qualified preventative care exam.
Unum | Critical Illness and Cancer
Unum Critical Illness insurance can help you protect yourself and your family from the unexpected cost of fighting a life-
threatening illness. You’ll receive a lump-sum cash payment upon diagnosis of a covered condition such as a Heart Attack,
Stroke, Kidney Failure and Cancer. There are no pre-existing condition limitations on this plan and the plan is offered
guarantee issue during your initial eligibility window only. This plan pays a $50 annual wellness benefit for covered
employees and spouses for getting a qualified preventative care exam.
Unum | Hospital Indemnity
Unum Hospital Indemnity insurance is designed to provide financial protection for covered individuals by paying a benefit
due to a hospitalization and in some cases, for treatments received for an accident or sickness. This plan covers
hospitalizations due to pregnancy, there is no pre-existing condition limitation on this plan. Lump sum benefits are paid
directly to the insured and do not coordinate with any other benefit payments. There are no pre-existing condition
limitations on this plan (except for pregnancy) and the plan is offered guarantee issue during your initial eligibility window
only. This plan pays a $50 annual wellness benefit for covered employees and spouses for getting a qualified preventative
care exam.
