Reliance Standard | Supplemental Benefits
You may purchase individual policies from MetLife. Your premiums are paid through payroll deductions on an after-tax basis.
Reliance Standard policies offer direct-to-the-policyholder cash payouts to help cover what other insurance doesn’t. All of
the Reliance Standard individual policies are portable, which means that you can keep them should you change jobs or
retire, with no increase in premiums.

Reliance Standard | Group Accident
You have the opportunity to apply for Group Accident Insurance through Reliance Standard. Group Accident pays benefits to
help you cover the unexpected costs associated with accidental death or injuries. Benefits are flat dollar payments and are
not based on medical expenses incurred. You may also apply for coverage for your spouse/registered domestic partner and
your dependent children.

Health Benefit Screening: Reliance Standard will provide an annual benefit of $75 per calendar year for taking one of the
eligible screening/prevention measures. MetLife will pay only one health screening benefit per covered person per calendar
year.

Reliance Standard | Critical Illness

You have the opportunity to apply for Critical Illness Insurance, a limited benefit policy offered through Reliance Standard.
This insurance is designed to help you offset the costs of certain covered critical illnesses such as stroke, permanent
paralysis, heart attack and end stage renal disease. When diagnosed with a covered condition, you will receive a lump-sum
benefit that you can use for expenses such as deductibles, copays, transportation, and so on.

You may apply for Critical Illness Insurance coverage with either a $10,000 benefit with no medical questionnaires or exams.
You may also apply for coverage for your spouse/registered domestic partner and dependent children. You must be enrolled
in a major medical plan in order to apply for Critical Illness Insurance. Please refer to the 2019 Monthly Rate Sheet.

Health Benefit Screening: Reliance Standard will provide an annual benefit of $50 per calendar year for taking one of the
eligible screening/prevention measures. MetLife will pay only one health screening benefit per covered person per calendar
year.
