Life Insurance & Services




         MetLife|Basic Life and AD&D
         Basic Group Term Life / Accidental Death and Dismemberment (AD&D) insurance is an important
         part of a complete benefits package. It provides protection to you and your beneficiaries. Please
         remember to update your beneficiary.

         MetLife|Voluntary Life and AD&D

         In addition to the company provided Basic Life and AD&D benefits, you may elect to purchase additional Term Life and
         AD&D insurance at discounted group rates provided by Met Life. You pay for this coverage with after-tax dollars through
         convenient payroll deductions. You may purchase coverage for yourself and your eligible dependents as follows:
         •   Employee: You may purchase coverage for yourself in increments of $10,000 up to a maximum benefit of $500,000, not
            to exceed 5 times your annual salary.
         •   Spouse:  If you buy coverage for yourself, you may also purchase coverage for your eligible spouse. Benefits for your
            spouse  are  available  in  increments  of  $5,000  to  a  maximum  benefit  of  $100,000  and  may  not  exceed  50%  of  your
            employee election.
         •   Child(ren): If you buy coverage for yourself, you may also purchase coverage for your eligible dependent child(ren) in
            the  following  amounts:  Age: 6  months to 26:  Flat  $1,000, $2,000, $4,000, $5,000 or  $10,000.  15 days  to  6  months:
            $1,000.

         Guarantee issue is a pre-approved amount of coverage that does not require you to provide proof of good health, and is
         available to you during your initial eligibility period (upon hire). Guarantee issue is available in the following amounts:
         •   Employee: $50,000
         •   Spouse: $25,000
         •   Child(ren): $10,000

         If you are no longer in your initial eligibility period, you may enroll in Voluntary Life and AD&D insurance anytime during the
         year as long as you provide proof of good health. To provide proof of good health, you will be asked to complete a health
         questionnaire and are subject to insurance carrier approval. MetLife may approve or decline coverage based on a review of
         your health history.

         MetLife | Will Preparation Services
         Will Preparation Services offer support and guidance to help you properly prepare the documents necessary to preserve
         your  family’s  financial  security.  This  service,  offered  by  MetLife  through  Hyatt  Legal  Plans,  fully  covers  attorney  fees  for
         preparing  or  updating  a  will,  living  will,  or  power  of  attorney.  Services  include  will  preparation,  in-person  or  telephone
         consultations with a participating plan attorney, unlimited access to update your will as long as you continue to be a member
         of our Life insurance plan through MetLife.

         MetLife | Estate Resolution Services
         Estate  Resolution  Services  provides  face-to-face  or  over  the  phone  probate  services  to  the  representative  (executor  or
         administrator)  of  the  deceased  employee’s  estate  and  the  estate  of  the  employee’s  spouse/domestic  partner.  Probate
         services include preparation of documents and representation at court proceedings needed to transfer the probate assets
         from the estate to the heirs, and completion of correspondence necessary to transfer non-probate assets. This plan covers
         participating plan attorneys’ fees for consultations or for the administrator or executor to discuss general questions about
         the probate process.
