1 Benefits of Customer eForms

 Easy access via OUR CCA intranet
 Real time and automated processing of Customer Creation, Maintenance and

     Change of Ownership information
 Provides a new Customer Number after successfully submitting a Customer

     eForms:
 Enabling Customers' order to be placed immediately via MYCCA
 Request Equipment via CRM
 Validation, drop down boxes and "smarts" built into the Customer eForm
 Option to complete Credit Application/Direct Debit Setup for new and existing

     Customers
 Incorporation of Liquor Licence validation as part of this process
 Customer experience in becoming a CCA Customer
 Welcome email sent to Customer
 MYCCA account set up for N&K Customers if requested

2 How to Access Customer eForms

Two options to find e-forms

a. Go to CCA intranet site – share-point, select the rocket symbol and locate
     e-forms

b. Log into SaM tap on the 3 horizontal lines to go to the Main Menu, tap on
     E-Forms. Ensure you are connected to VPN
