Reporting Procedures
Where possible, the following information should be documented and provided to Emergency Services personnel at handover:
• Date, time and location of incident
• Name, DOB and Address
• Next of Kin’s name and contact number
• Signs and Symptoms
• Allergies
• Medications
• Past / Present Illness (e.g. diabetes, asthma, heart
issues)
• Last Meal
• Events (what happened)
• Observations/vital signs (pulse, breathing rate, pupil
size)
• First Aiders name and contact number
Where the incident occurs in a work environment, it is imperative the incident is reported and accurate details of the incident recorded as per the specific workplace’s policies and procedures
