2019 expansion plans:
Yearly goals for care 2018 initiatives included:
• Solidifying our faith community relationships. We spent time this year deepening and sharpening our health ministries, and our relationships with our liaisons
• Revamping the patient referral process. We have now streamlined the referral process to make it easier for health professionals to refer to us
• Establishing monthly health observance focuses, a newsletter that is sent out for each liaison to use in their congregation
• Creating the Connector role throughout the state, part-time contractors who represent Faith Community in their area

Updating and republishing our How to Start a Church Health Ministry guide in order to strengthen our faith community health ministries
