Who is a Faith Community Health caregiver?
Faith Community Health caregivers are trained volunteers who support the patient in their health goals by offering companionship for one hour per week.
Caregivers abide by our four principles of care:
 • Right door: providing education for patients on the most appropriate healthcare treatment location
• Ready to be treated: helping people prepare to share concerning symptoms, medication, and home remedies with their healthcare provider
  • Right time: providing patients information to recognize symptoms early
• Reassurance: treating people with compassionate care, assuring them
they are not alone
  Caregiver training includes:
• Learning the four priniciples of care, above
• Maximizing productivity in home visits
• Communication and listening skills
• Basics of mental health
 • Setting boundaries with patients
• Use of situation-specific reference cards
 Caregiving is provided to patients who:
• Live alone and do not have a support system (this could apply to an elderly couple as well)
• Are undergoing a crisis and could use additional forms of support
 • Are in need of social and/ or spiritual support
• Have a safe living environment, do not have aggressive tendencies, abusive
behavior, or drug habits
• Could use assistance following plan of care from a non-medical perspective (assistance with food/ meals, transportation, etc.)
• Will consent to a non-medical volunteer, and have time
for volunteer to make visits for one hour per week
