Voluntary Critical Illness Covered Conditions
Critical illness insurance, available through MetLife, is designed to help (paid at 100% of
elected amount unless
you offset the inancial effects of a catastrophic illness with a lump sum otherwise noted below)
beneit if you or a loved one are diagnosed with a covered critical illness. X Cancer
The critical illness beneit is based on the amount of coverage in effect on
the date of diagnosis of a critical illness or the date treatment is received X Heart attack
according to the terms and provisions of the policy. X Coronary artery bypass
X During beneits enrollment, you can elect this coverage without graft
medical questions X End stage renal (kidney)
X Employee: $15,000 or $30,000 failure
X Spouse/domestic partner: 50% of the employee’s initial beneit X Major organ transplant
X Child(ren): 50% of the employee’s initial beneit X Stroke
X The maximum amount you can receive is 3 times the amount of your X Alzheimer’s disease
initial beneit
X Coverage is portable which means you can take this plan with you if X Occupational HIV
you no longer work for the company X 22 additional conditions
X Your rates lock in at your current age and do not increase as you age paid at 25% of your initial
benefit; please see your
certificate for a complete
Wellness Benefit list
This beneit pays $75 per calendar year per insured individual if a covered
health screening test is performed, including blood tests, chest x-rays,
stress tests, mammograms, and colonoscopies. A full list of covered tests
will be provided in your certiicate.
