Apricity Benefits Overview
VOLUNTARY DISABILITY INSURANCE
Consider what would happen inancially if you became disabled and could no longer work due
to an injury or illness. It’s likely that it would be a inancial challenge to replace enough income to
meet your monthly expenses. That’s why the Company offers voluntary disability coverage to all
eligible employees.
You pay the full cost of this valuable coverage through post-tax payroll deductions. Coverage is
available through MetLife at discounted group rates.
Short-Term Disability (STD)
STD will help replace a portion of your salary in the event of a covered pregnancy, illness, or
accident. Beneits are provided for both total and partial disability.
The maximum beneit is up to 60% of your base weekly pay with a weekly maximum of $1,500.
Beneits begin after 7 days of continuous illness or injury and may continue for up to 13 weeks.
Long-Term Disability (LTD)
If you remain disabled for longer than 13 weeks, you may be eligible for LTD beneits. LTD provides
60% of your pre-disability earnings to a maximum beneit of $6,000 per month. Beneits are offset
by other income sources you may receive such as state disability, worker’s compensation, Social
Security, retirement, and severance beneits. You must notify MetLife if you begin receiving other
income beneits.
Pre-Existing Conditions
The plan will not pay a beneit for 12 months after the effective date for a pre-existing condition
that occurred within 3 months prior to the effective date for STD and LTD.
You will be considered to have a pre-existing condition if:
You received medical treatment, consultation, care, or services, including diagnostic
measures, or were prescribed drugs or medicines in the 3 months prior to your effective date
of coverage; and
The disability begins in the irst 12 months after your effective date of coverage.
