Apricity Benefits Overview
LIFE AND AD&D INSURANCE
Company-Paid Group Life Insurance
Life insurance is an important part of your inancial security,
especially if others depend on you for support. That’s why the
plan provides Company-paid group life insurance through
Lincoln Financial Group at no cost to you. All active, full-time
employees receive a Company-paid group life insurance
beneit that is equal to one times (1x) your annual base salary up
to a maximum of $100,000.
Coverage Specifics
You must name a beneiciary—the person or persons who
receive your life insurance beneit in the event of your
death.
Your beneit reduces to 65% at age 70 and to 50% at age
75.
You can convert your Company-paid group life insurance
beneit into an individual policy upon your retirement or
separation of employment.
Voluntary Life and AD&D Insurance
You have the option to purchase voluntary life insurance for
yourself in amounts from $5,000 up to a maximum of $500,000.
Conversion Feature Coverage is available in $5,000 increments. You must submit
