Disability leave which qualiies as Family and Medical Leave will be

counted against an employee’s 12-week FMLA entitlement. During
this time, all FMLA job protections apply. Insurance beneits will
continue during the short term disability time frame. Insurance beneits
will continue; however, you will be responsible for your portion of
the beneit premium (if applicable). After the short term disability
timeframe, employees will be responsible for the entire cost of any
insurance they wish to continue.


In no event may an employee exceed the above-speciied allowance in
any 12-month period. In addition, once the full amount of disability

leave has been exhausted during the 12-month period, no additional
disability is provided until said employee has returned to work and been
on the active payroll for a period of one year.


Maternity and child adoption are considered extended illness for the
issue of short term disability.


Reservation of Rights
Americare reserves the right to amend, modify, or terminate this policy
at any time and without advance notice, subject to the requirements of

applicable law. Please keep in mind this STD salary continuation policy,
any changes to it, any payments to it, or any payments to you under its
terms, does not constitute a contract of employment with Americare
and does not give you the right to be retained in the employment of
Americare. No one speaking on behalf of this STD salary continuation
policy or Americare can alter the terms of this policy.
