2022 Enrollment Process


DAWSON’s Benefits Enrollment is
conducted using the ADP online
enrollment system.





REQUIRED DOCUMENTATION & IDENTIFICATION


If you plan to add a spouse or dependent children to DAWSON’s benefits,
you must provide a marriage certificate for participating spouses and birth
certificates for all participating children. If you cover a domestic partner, you will
be required to complete an affidavit to confirm their tax status. You will need
to provide valid Social Security Numbers and/or taxpayer IDs for each enrolled
dependent. DAWSON is required to provide names and Social Security Numbers
(when possible) to the federal government for each individual enrolled in medical
coverage. You can upload your documentation through workforcenow.ADP.com
as you complete your enrollment.




BENEFITS ELIGIBILITY



COVERING YOURSELF
You may enroll in the benefits program if you are a regular, full-time employee
who is actively working a minimum of 30 hours per week (or 20 hours per week
if you work in Hawaii).




COVERING YOUR FAMILY MEMBERS
Eligible dependents generally include your legally married spouse, same or opposite
sex domestic partner, and children up to age 26. Children may include natural,
adopted, stepchildren, or children through court-appointed legal guardianship.

If you cover a domestic partner, you must complete a domestic partner affidavit
that is available on workforcenow.ADP.com as you make your enrollment
elections. If your domestic partner is not your dependent for tax purposes,
DAWSON will impute the value of the portion of the premium DAWSON pays
on behalf of your domestic partner and that amount is subject to certain taxes.
