Understand your coverage
At ConnectiCare, we help our members get the most out of their health plan by providing easy access to
coverage information. Simply log onto www.connecticare.com/members and click on My Plan then
My Benefit Information to see all the coverage you are eligible for. Contact us at 1-800-251-7722 for more
information, or via e-mail at info@connecticare.com. Once you‘re a member, you can e-mail us through our
secure messaging online.
Benefit plan information*
At www.connecticare.com/members, you can access plan benefits information, including:
• Medical and pharmacy claims history;
• Online Benefit Summary;**
• Health Care Financial Summary;
• Secure messaging from case managers and Member Services representatives; and
• Many more tools and resources!
*Your privacy and the security of information are our top priority. First, be sure to read our privacy policy on our website to understand the steps we take to
keep your information private. Second, we have stored information about you, including your name, address, your doctor and benefit coverage information in
a secure database. To gain access to many features of the member site, and to work with your information, you will need to register and log onto the site with
a user name and password that you will create. However, if you do not want to register to use these services, and would prefer us to delete your information
from our secure database, please notify us in writing at the following address:

benefits in the Membership Agreement/Certificate of Coverage and lists the copayments, deductibles and coinsurance levels that a member is required
to pay for covered health services, as well as benefit maximums. For more information, refer to your ConnectiCare Membership Agreement/Certificate
of Coverage or contact your benefits manager. The Membership Agreement/Certificate of Coverage will prevail for all benefits, conditions, limitations
and exclusions.
Please note: In some cases, the Prescription Drug Benefit Summary may not be available online. Please contact your benefit manager for more information,

Through Leapfrog, find hospitals that have disclosed information about safety practices.

Learn more about providers from the Connecticut Department of Public Health.
