For Anthem members in Plans 1 or 2, a Live Health Online consultation charge is treated the
same way as a traditional office visit: $20 Copay (Plan 1) and $25 Copay (Plan 2). The
copay does not count towards the deductible, but does count against the annual Out-of-
Pocket Maximum.

For Anthem members in Plans 3, 4 or 5, the full cost of the consultation charge - $59 or less,
accumulates towards deductible and out-of-pocket maximum.

Live Health Online is not a replacement for your primary care physician (PCP). It is important
to maintain a relationship with your PCP, and to continue seeking preventive and routine care
through that provider.
