Benefits Open Enrollment

Welcome to the PetVet Care Centers 2022 Short Plan Year Benefits Program!

PetVet is committed to supporting your health and wellbeing by offering choice and a wide range
of benefits available to you and your families. We invite you to consider your benefit elections
during Open Enrollment, which opens June 7, 2021 and will close June 20, 2021 at 11:59 p.m.
Eastern Daylight Time.

Your ‘Short Plan Year’ elections will be in place through December 31, 2022.

If one or more of the following is true, you are required to review and make an election for each
benefit offered.

 You want to elect a Health Savings Account (HSA) for the Period 7/1/2022 – 12/31/2022
 You want to elect a Flexible Spending Account (FSA) for the Period 7/1/2022 – 12/31/2022
 You want to cancel a benefit election or make a new benefit election
 You want to change coverage tiers or add/delete dependent(s)
 You want to make any other change to any benefit in which you are currently enrolled

If none of the above is true and you take no action, you will be automatically re-enrolled in the

benefit programs for which you are currently enrolled . Importantly, per IRS guidelines, HSA and
FSA accounts are excluded from auto re-enrollment eligibility.

1 Open Enrollment is ‘Passive’ which means your current elections (except FSA & HSA) will carry forward into the new plan year , unless you log into
Empyrean and make changes.
