Benefits Eligibility
Who is benefits eligible?
Full-time employees, regularly scheduled to work at minimum 30 hours per week
Who are eligible dependents?
The following dependents are eligible for health care, life and disability coverage:
Legal Spouse

Dependent children up to age 26 (unless disabled)
Domestic Partners – affidavit required
What information will I need to complete the enrollment process?
Legal names, dates of birth and Social Security Numbers of any dependent you want to cover
The benefits selected during Open Enrollment will be effective on July 1, 2022 and will remain
in effect through December 31, 2022 2
1 Medical, dental, and vision coverage terminates upon plan year renewal for dependents who turn 26.
2 Benefits changes can be made after the Open Enrollment period closes if a valid, qualifying event is experienced. If hired after the
Open Enrollment period closes, elected benefits will begin the first of the month following 30 days of employment.
