            BENEFIT BASICS


            ELIGIBILITY
            Regular employees who work at least 30 hours per week are eligible for the benefits described
            in this guide.

            Most benefits are effective on the first day of the month following 30 days of employment
            as a regular full-time employee. The following dependents are also eligible:




            +   Your legal spouse       +   Your domestic partner      +   Your children up to age 26
            ENROLLING FOR BENEFITS
            The online Monster Employee Portal (UKGPro) allows you to enroll, make changes, or
            waive coverage for: medical, dental, vision, Health Savings Account, Flexible Spending
            Accounts, supplemental life insurance, whole life, critical illness and accident insurance,
            and legal services.



            OPEN ENROLLMENT
            You may make changes once a year during open enrollment using the Monster Employee Portal
            (UKGPro). Access the online portal: https://n22.ultipro.com. The first time you log in:

            +   Your username is your first initial, last name and last four digits of your Social Security number.

            +   Your temporary password is your date of birth MMDDYYYY.

            +   The system will prompt you to select a new password.


            LIFE EVENT—MAKING CHANGES TO YOUR BENEFITS
            Generally, you may only make or change your existing benefit elections as a new hire or during
            the annual open enrollment period. However, you may change your benefit elections during the
            year if you experience a qualified life event such as:

            +   Marriage, divorce or legal separation

            +   Birth or adoption of a child

            +   Loss or gain of other coverage by the employee or dependent

            +   Eligibility for Medicare or Medicaid
            You have 30 days from the qualified life event to make changes to your coverage.
            Depending on the type of event, you may need to provide proof of the event, such
            as a marriage license. If you do not make the changes within 30 days, you will have to
            wait until the next open enrollment period (unless you experience another qualified life event).
