Benefits Eligibility

We hope this benefits guide is a helpful tool as you make your benefit choices. Review it carefully, as it gives specific
information about each of the options available to you.

Who is eligible for health care, life and disability benefits?

• Core Associates: Regular and full-time associates who work at least 30 or more hours per week.
• Variable-hour associates who meet the following:
» A variable-hour associate may include a part-time, seasonal or temporary associate.
» Benefits eligibility will be reviewed during the first year of employment. This is called the initial measurement period for
the purpose of determining benefits eligibility.
» If a variable-hour associate averages at least 30 hours per week during their initial measurement period, they will
become benefits eligible.
» The benefits effective date for associates who become benefit eligible after review of the initial measurement period
will start after an eight-week administrative period.
» The associate will remain benefit eligible for the duration of the initial stability period, which is 12 months following
the effective date of coverage.
• Ongoing eligibility for variable-hour associates who have been with the Company one year or longer:
» There will be an annual review to determine benefits eligibility.
