Wellness and Tobacco




       We care about your health and well-being. We believe wellness should be an integral part of our benefits program to all health plan covered employees.


       Wellness Discount
       To be eligible for the Wellness Discount for 2023-24, employees must have completed their preventive visit between 8/1/2022 and 7/31/2023. If the preventive
       visit is not completed by 7/31/2023, the employee will receive the Non-Wellness rates for the remainder of the plan year.

       As an Alternative Standard, if it is unreasonably difficult due to a medical condition or if it is medically inadvisable for you to achieve the standards or alternative
       standards for the reward under this component of the program, please contact Human Resources and we will work with you to develop another way to qualify
       for the reward. (Alternative Standard for Tobacco Discount is listed below).

       Employees hired after 8/1/2023 will have to certify if they wish to participate in the Wellness plan once eligible for benefits. The employee will receive the
       Wellness discounted rates until Open Enrollment on 8/1/2024. You will be required to complete a preventive visit prior to 8/1/2024 to continue receiving the
       Wellness discounted rates. Otherwise, it will result in the Wellness Discount being removed for the 2024-2025 plan year.


       Tobacco-Free Discount
       To be eligible for the Tobacco-Free Discount for 2023-24, employees must:
       •  Attest during open enrollment 2023 to being tobacco-free as of May 1, 2023.
       •  As an alternative standard if you are tobacco user, employees will be eligible for the tobacco-free discount if you complete the Tobacco Wellness Program
         with BlueCross BlueShield of Tennessee by 10/31/2023. You will need to attest during open enrollment that you will complete the program. The program
         will open on 8/1/2023. Details to follow.

       Employees hired after 8/1/2023 will need to attest if they are tobacco-free upon enrollment in the health plan. If employees are tobacco users, they may
       qualify as being tobacco-free by enrolling and completing the Tobacco Cessation Program with BlueCross BlueShield by calling 866-498-9806.
       If you do not enroll in the Tobacco Cessation program and complete it, you will not be eligible for the tobacco-free discount.
