Benefit Overview




                                                                   Insurity offers a comprehensive suite of benefits to promote
                                                                   health and financial security for you and your family.
                                                                   This guide provides an overview of your Insurity Benefits
                                                                   package. Actual benefit details are subject to the
                                                                   provisions and limitations of the agreements between
                                                                   Insurity and its providers. Please review the guide and
                                                                   agreements carefully so you can choose the coverage that
                                                                   is right for you and your family.
                                                                   Our Medical, Dental and Short-Term Disability plans are
                                                                   self-funded. This means that Insurity uses our own funds
                                                                   to pay the claims for these benefits. Throughout this guide,
                                                                   we include information and tips on ways to stay healthy.
                                                                   When our company is in good “health” we all save!

                                                                   Benefit Basics

                                                                   The following benefits are available to regular, active
                                                                   employees who work 20 hours or more per week on
                                                                   average and meet all other eligibility requirements.
                                                                   Temporary employees, including interns, are not eligible
                                                                   for benefits regardless of the number of hours worked
                                                                   per week.
                                                                   You may also enroll your eligible dependents, which
                                                                   may include:
                                                                   O   Your legal spouse
                                                                   O   Your eligible child(ren). Covered dependents can
                                                                      remain on your plan until the end of the calendar
                                                                      year in which they turn age 26
                                                                   O   Other legally required dependents
                                                                   Once your benefit elections become effective, they
                                                                   remain in effect until the end of the plan year. You may
                                                                   only change coverage if you experience a Qualifying
                                                                   Life Event (QLE).
