Disability Insurance
Disability insurance provides income replacement should you become disabled and unable to work due to a
non-work-related illness or injury. Ampact provides eligible employees with disability coverage at no cost as
shown below. Coverage is automatic; you do not need to enroll.
Coverage Benefit
Short-Term Disability • Weekly Benefit Amount - 60% up to $1,000
• Benefit Waiting Period – 7 days for accident
and 7 days for sickness
• Maximum Benefit Period - 12 weeks
Long-Term Disability • Covers 60% of your base annual earnings, to
a $5,000 monthly maximum
• Benefit begins after 90 days of disability
• Pre-Existing Condition Limitation – may not
be eligible if you have received treatment
for a condition within 3 months prior to your
effective date under this policy until you
have been covered under the policy for 12
months.
